This manual covers the complete monthly workflow for LIFE add-on compliance,
inspection preparation, and AI comment generation.

System Overview

CAMP LIFE is a system designed to fulfill the LIFE add-on "utilization" requirement. By importing monthly LIFE data, the system automatically handles at-risk detection, multidisciplinary review records, PDCA documentation, and inspection document output โ€” all in one workflow.

Enter the email address and password provided at the time of contract.

Upon successful login, the Operations Dashboard will be displayed.

New LIFE System (NHIA-Operated)

On May 11, 2026, LIFE operations were transferred from the MHLW to the National Health Insurance Association (NHIA / ๅ›ฝไฟไธญๅคฎไผš). The migration deadline is July 31, 2026. Please confirm the changes that affect CSV import into CAMP LIFE.

Failure to complete migration may result in loss of LIFE add-on eligibility. The old LIFE system will shut down completely on September 1, 2026. Download all past feedback data as PDFs from the old system before September 1.

Migration Steps (Summary)

Install the Digital Certificate

Import the digital certificate distributed by the NHIA helpdesk onto each device used for LIFE. Once installed, one-time passcodes are no longer required.

Request Data Migration from Old System

Log in to the old LIFE system, select "Data Migration," agree to the terms, and click "Execute Migration." Account basic information will be transferred to NHIA.

Re-register Resident Information

For active residents, re-register their information in the new system by importing a CSV file exported from your care management software.

Back Up Past Data from Old System

While the old system is still accessible (read-only), download past feedback data as PDFs and save them locally. Deadline: September 1, 2026.

โ‘  LIFE Monthly Data Import

Import the CSV file exported from the MHLW LIFE system into CAMP LIFE each month.

Step 1

Download CSV from LIFE System

Log in to the MHLW LIFE system, go to "External Interface" โ†’ "Data Download," and download the Scientific Care Promotion CSV file.

File name example: LIFE_export_202601.csv
Step 2

Open the Import Screen

Click "โ‘  Import monthly LIFE data" on the dashboard, or click "Monthly Import" in the top navigation.

Step 3

Select and Upload the CSV File

Click "Select File," choose the downloaded CSV, then click "Import." The system automatically checks for BOM, encoding issues, and record counts.

โš ๏ธ Import errors: Verify that the CSV is saved as Shift-JIS encoding. UTF-8 may cause BOM errors.

How to Retrieve LIFE Feedback

The essence of LIFE add-on "utilization" is not just submitting data, but using the feedback provided by the government to drive PDCA improvement cycles. Receiving feedback, conducting multidisciplinary review, reflecting it in care, and documenting everything โ€” this is what inspections verify.

โš ๏ธ Past feedback data from the old LIFE system will NOT be migrated to the new system.
All feedback data accumulated in the old (MHLW-operated) LIFE system must be manually downloaded as PDFs before September 1, 2026 (old system shutdown). After that date, access becomes permanently impossible.

Feedback Retrieval Steps (NHIA-Operated LIFE)

Log In and Check Notifications

Log in to https://top.life-kkh.jp/ using your digital certificate. When new feedback data is ready, a notification appears in the "Announcements" section of the system.

Open "Feedback Download" Screen

From the home (menu) screen, click "Feedback Download" to access the feedback download screen.

Check the Status of Each Report

Verify the "Status" shown for each feedback report in the list.

Download "Ready" Reports

Select "Ready" feedback reports and download them to your local environment. This download operation is available to both system administrators and "operator" staff accounts โ€” enabling multidisciplinary sharing.

Import into CAMP LIFE for PDCA Use

Downloaded feedback PDFs can be imported via CAMP LIFE's "Supplemental: FB PDF Import" screen. Combine with CAMP LIFE's analysis features to drive at-risk detection, multidisciplinary review, and PDCA records.

Handling CSV Import Errors

Checking and Resolving CSV Import Errors

โ‘  In the NHIA LIFE system, go to "External Data Import" โ†’ check "Data Import Results"

โ‘ก Click "Export Error CSV" โ†’ download a detailed error CSV listing all submission failures and field issues

โ‘ข Refer to the error codes and field issues listed in the error CSV; correct them in your care management software and re-export

โ‘ฃ Re-import the corrected CSV into CAMP LIFE to complete the process

โš ๏ธ Back Up Past Feedback Data from Old LIFE System (Deadline: August 31, 2026)

All feedback data accumulated in the old (MHLW-operated) LIFE system will NOT be transferred to the new system.
After the old system shuts down, access to past data becomes permanently impossible.
Nearly 5 years of resident status history โ€” a critical information asset โ€” will be lost.

Act Now:
โ‘  Log in to the old LIFE system (MHLW-operated, currently in read-only mode)
โ‘ก Download all past feedback reports (Facility Reports and Individual Reports) as PDFs
โ‘ข Save them to your facility's server or local PC

The old system is currently accessible in read-only mode. Complete this before August 31, 2026.

LIFE CSV Export by Care Software

To submit data to LIFE, export a CSV from your care management software and upload it to the new LIFE system (NHIA). The exported CSV can also be imported into CAMP LIFE for analysis. Steps vary by vendor.

๐Ÿ“Œ CSV Spec: From May 11, 2026, only v0310 (May 11, 2026 updated) compliant CSVs are accepted. Ensure your software is updated to the latest version before exporting. ZIP format bundles are also accepted.

Importing the CSV into CAMP LIFE

Import Any Vendor's CSV into CAMP LIFE

CSV files from any of the above vendors can be imported into CAMP LIFE's monthly import screen.

โ‘ข BOM, encoding, and record count are automatically checked. Import completes.

โš ๏ธ Note: Submitting to the LIFE system (NHIA) and importing into CAMP LIFE are two separate operations. Submit to LIFE via your care software, then separately import into CAMP LIFE for analysis and evidence generation.

Click the "Time-Series" button next to the target resident in the at-risk list.

Click "Generate AI Comment"

Click the "๐Ÿค– Generate AI Comment" button at the bottom of the screen. The comment will be generated within a few seconds.

๐Ÿ”’ Privacy: Data is pseudonymized before being sent to AI. No personally identifiable information (name, DOB, address) is transmitted to AI.

Review, Edit, and Save

Read the generated comment carefully and revise it based on the resident's actual condition, wishes, and medical context. Click "Save" when complete.

Final judgment must always be made by staff. Do not submit AI-generated text without professional review and modification.

Manager "Confirm" Action

After reviewing all content, a manager-level account must click "Confirm." Confirmed records become immutable and are stored as official evidence.

The confirmer's name and timestamp are automatically recorded, serving as evidence during operational guidance inspections.

This confirmation record is central to proving the LIFE add-on requirement: "PDCA cycle implementation."

Preparing for Operational Guidance Inspections

Once you receive inspection notice, follow these steps. If you have been outputting documents monthly, preparation takes under 30 minutes.

Print 12 Months of Evidence Packs

In the document output screen, select a 12-month period and output all Evidence Packs as PDFs for printing.

Output the Latest Inspection Submission Pack

Output the current month's Inspection Submission Pack โ€” a comprehensive set integrating status changes, multidisciplinary review, PDCA, and documents.

Prepare to Explain "How You Utilized LIFE"

AI Usage Policy

Principle

Judgment by Code | Writing by AI | Final Decision by Humans

Judgment by Code: At-risk assessment, level classification, and 6-domain scoring are performed entirely by program code. AI has no involvement in assessment logic.

Writing by AI: AI generates professional and care-plan commentary text based on assessment results.

Final Decision by Humans: All AI-generated content must be reviewed, revised, and confirmed by a qualified care professional. AI output is reference material, not a medical diagnosis.

Privacy

Resident Personal Data Is Never Sent to AI

Before transmitting data to AI, the system automatically pseudonymizes all records. Real names, dates of birth, addresses, and other personally identifiable information are never sent to AI.
